In consideration of permission to use, today and on all future dates, the services of T and J Fitness Pt / Teresa Edwards / Jonathan Edwards and to participate in weight training, cardiovascular training, Bootcamp, classes and other physical activities, the Client, on behalf of himself or herself, his or her heirs, personal representatives, and assigns, does hereby release, waive, and discharge all T and J Fitness Instructors / Teresa Edwards and Jonathan Edwards from liability for any and all claims arising from the clients own negligence, the negligence of others, or the released parties’ negligence originating from the clients participation with T and J Fitness Pt. This agreement applies to: 1) personal injury (including death) from accidents or illnesses arising from the participation in physical activities including, but not limited to, organised activities, classes, observation, personal training, bootcamp, online personal training, nutritional advice or equipment; and 2) any and all claims resulting from the damage to, loss of, or theft of property.
The Client certifies that he or she knows of no medical problem (except those listed on the PARQ) that would increase his or her risk of illness and injury as a result of participating in any physical training programme given by T and J Fitness Pt / Teresa Edwards and Jonathan Edwards or taking part in any T and J Fitness Pt class. The Client understands that he or she has been advised to consult a physician prior to commencement of a physical training programme to ensure his or her state of wellness to participate in such strenuous activities. Client agrees to indemnify and hold the released parties harmless for any loss caused by the client for which the released parties are held liable including solicitor’s fees and arbitration costs.
I give T and J Fitness Pt the right to take video footage and/or photographs of sessions or events.
I understand that some videos and/or photos could be posted on various social media sites, websites and/or could be sent out for email marketing promotions.
T and J Fitness will own the copyright to all photo and video.
To book a T and J Fitness Pt class is very simple, just go to our website and use the booking link on the appropriate page, fill in your details and send. Payment can be made by cash, bank transfer or direct debit.
All block bookings for bootcamp ( 4 weeks), and Boxercise ( 6 weeks) must be paid for in full prior to commencement. All amounts are non refundable, any sessions must be booked and used within the 4 week or 6 week period.
T and J Fitness Pt understands that emergencies do happen, so we allow every client two short notice cancellations in a six month period free of charge. This only applies if T and J Fitness Pt is notified of cancellation by text or email prior to the session start time. Non attendance without prior notification will be charged in full and the emergency waiver of fees will not stand. The cost of the approved cancelled sessions will be deducted from your next block booking. There is no cash alternative.
If for any reason you are late, please be aware that the session will continue until the agreed finish time. For example , if you have booked an hour session and you arrive 10 minutes late, then the session will last 50 minutes.
Should you arrive more than 30 minutes after the start time, the session will be cancelled and charged as a no show.
